By proceeding with this form you acknowledge that to claim a product under Cooke Industries’ manufacturer warranty, you will need to return the defective item for assessment. The product must also be within the warranty period.

Please note that all postage costs incurred for the return will be the responsibility of the claimant.

Additionally, you must include within the postage bag alongside the claimed product, either:

a printed copy of this submitted form, or at the bare minimum
the ticket reference code.

Both of which will be generated and sent to your designated email address.
